In-Home Cardiac Rehab

Personalized Cardiac Monitoring

Every session includes individualized monitoring to ensure exercise remains safe and effective. Our exercise physiologists track heart rate, blood pressure, symptoms, and perceived exertion to guide each patient’s progression and maintain clinical safety.

Lifestyle Guidance

We help patients build sustainable habits that support long-term heart health. This includes guidance on daily movement, walking routines, and practical strategies to stay active outside of supervised sessions.

Exercise Prescription

Each patient receives a customized exercise program based on their condition, functional capacity, and recovery goals. Programs include aerobic training, functional strength exercises, and progressive conditioning designed to safely improve cardiovascular health.

Safety

Patient safety is our highest priority. All sessions are delivered by trained exercise physiologists who closely monitor symptoms and vital responses to ensure exercise remains appropriate and well tolerated.

How NEXUS Works

  3. In-home rehabilitation sessions
    Patients receive supervised exercise and monitoring in their home.

  4. Ongoing progress tracking
    Programs are adjusted as patients improve.
