Our process is simple, safe, and structured to maximize recovery outcomes.

Step 1 – Physician Referral
Referring providers submit a quick referral form via our website. A physician signature confirms the patient is medically stable and cleared for home-based cardiac rehabilitation. If needed, we’re happy to discuss the patient’s care plan before enrollment.

Step 2 – Initial Home Assessment
A trained exercise physiologist visits the patient’s home. Baseline measurements include heart rate, blood pressure, oxygen saturation, and functional capacity. Patient goals, limitations, and preferences are assessed to create a personalized program.

Step 3 – Personalized Rehab Plan
Patients receive a customized exercise program designed for safety and effectiveness. Sessions include cardiac monitoring and progress tracking. Lifestyle guidance is incorporated, including nutrition, stress management, and habit-building strategies.

Step 4 – Ongoing Monitoring and Progress Reporting
Exercise sessions are conducted in the home and monitored with FDA-cleared technology. Physicians receive regular progress updates, keeping them informed of the patient’s recovery. Adjustments are made as the patient improves to ensure safe progression and optimal outcomes.

Step 5 – Completion and Follow-Up
Once the program is complete, patients receive a summary of outcomes, including improvements in functional capacity and adherence. Physicians are provided a final report with recommendations for ongoing cardiovascular health. Optional continued support is available to maintain long-term lifestyle changes.
