NEXUS was created after witnessing firsthand how difficult it can be for patients to access the rehabilitation they need after a major health event.
After suffering a stroke, my grandmother struggled to access the rehabilitation services necessary for her recovery. During this time, our family learned that her physician was not aware of cardiac rehabilitation as an option that could support her recovery. Like many families, we quickly realized that even when rehabilitation can significantly improve outcomes, many patients face barriers that prevent them from participating.
Transportation challenges, limited program availability, and lack of awareness about rehabilitation programs often stand in the way of recovery.
NEXUS was created to address this gap, delivering safe, structured cardiac rehabilitation directly to patients in their homes.

Founder, NEXUS Cardiac Rehabilitation LLC
Braxton Walker is an ACSM Certified Exercise Physiologist and graduate of Georgia State University, where he earned a Bachelor of Science in Exercise Science.
His experience includes work in clinical exercise testing, biomechanics research, and industrial injury prevention. Through his work in health and rehabilitation, Braxton became increasingly aware of the significant gap between patients who are referred to cardiac rehabilitation and those who are actually able to participate.
NEXUS was founded to help close that gap by delivering high-quality cardiac rehabilitation directly to patients where they are — at home.

NEXUS is currently a cash-based program, which allows us to provide flexible, high-quality, home-based cardiac rehabilitation without the restrictions of insurance coverage.

Maintain consistent communication and progress reporting to physicians
While NEXUS is not currently billed to insurance, we plan to work with insurance providers in the future. Our goal is to make home-based cardiac rehabilitation more widely accessible while maintaining the high level of personalized care that our patients and physicians value.

NEXUS provides structured, home-based cardiac rehabilitation designed for patients recovering from heart attacks, cardiac procedures, heart failure, and other cardiovascular conditions. Our program brings supervised rehabilitation directly into the patient’s home, improving access and participation.

Patients who have experienced conditions such as Myocardial Infarction, Percutaneous Coronary Intervention, Coronary Artery Bypass Grafting, Heart Failure, or certain valve procedures may be eligible. A physician referral and medical clearance are required before beginning the program.

NEXUS is currently accepting patients within Atlanta metropolitan area for our initial cohort of home-based cardiac rehabilitation participants. At this time, visits are scheduled on weekends to help accommodate patients who may have difficulty attending traditional weekday rehabilitation programs.
As NEXUS grows, we plan to expand both service availability and scheduling options to reach more patients across the region.

Each session includes monitoring of heart rate, blood pressure, oxygen saturation, and cardiac rhythm when appropriate. Sessions are conducted by a trained exercise physiologist using clinical monitoring tools to ensure rehabilitation is delivered safely and effectively.

Traditional cardiac rehabilitation is typically conducted in hospital or clinic settings. NEXUS brings rehabilitation directly into the patient’s home, removing common barriers such as transportation, travel time, and scheduling conflicts. This approach can improve patient participation and adherence.

Yes. NEXUS communicates with referring providers and can share progress updates, program completion summaries, and key outcomes to help physicians stay informed about their patient’s recovery.
